Description: Health Center Connections, the HCCN organized and supported by the Community Care Network of Kansas, created this survey to ask clinicians at participating health centers about their work environment, productivity, health IT in use, and health IT interventions. The intent of the survey is to collect information from clinicians that can identify opportunities to reduce provider burden through the use of health IT.
